    Joan’s Home is the primary sponsor of Our Legacy Group Home, which is a trauma-informed, youth-centered group home launching in Cleveland, Ohio, to serve at-risk youth ages 12–18 facing homelessness, family separation, or involvement in foster care or the juvenile justice system.

Who We Are

Joan’s Home is a trauma-informed, youth-centered supportive living home located in Cleveland, Ohio. Founded in 2024 after the passing of our founder’s mother, Joan, we were created from a place of love, healing, and purpose. Our mission is to provide safe, stable housing and holistic support for youth ages 12–20 who are experiencing homelessness, family separation, or system involvement.
More than just a place to live, Joan’s Home is a place to grow. Through mental health services, life skills training, education support, and workforce readiness programs, we empower young people to heal, build confidence, and prepare for independent adulthood.
We believe every young person deserves the chance to thrive. At Joan’s Home, we provide the care, consistency, and community they need to do just that.

How We Help

  • We understand the need for confidentiality and privacy in such sensitive circumstances, and therefore, all services are provided with the utmost respect and discretion. Our goal is to provide a safe haven where teens can heal, find strength, and ultimately, break free from whatever battles life has thrown at them. Joan's Home was created to provide a safe and secure transitional housing for our residents, while also offering them an extensive care plan to aid in their journey towards a brighter future.

  • At Joan’s Home, we offer more than a roof—we provide a foundation for hope, healing, and growth.

  • Youth are placed with us through partnerships with local agencies and case management teams who identify individuals ages 12–18 in need of a safe, supportive living environment. Once placed, each resident receives an individualized care plan tailored to their emotional, educational, and developmental needs.

  • We surround our youth with a nurturing, trauma-informed environment and provide access to:

  • -Mental Health Support & Counseling

  • -Life Skills Training (budgeting, cooking, hygiene, and more)

  • -Educational Support & School Re-engagement

  • -Workforce Development & Job Readiness

  • -Social Skills Workshops & Personal Development

  • Through our community outreach programs, youth also engage in:

  • -Volunteer opportunities with local organizations

  • -Mentorship programs with positive adult role models

  • -Community-building events and field trips

  • -Service-learning projects that foster civic responsibility
